  Immunization Clinics are conducted at the Adams County Health and Human Services Building - 108 East North Street, Friendship, WI 53934

  • Please call (608)339-4505 to make an Appointment
  • Please Bring Records of Child's Immunizations
  • Parent or Guardian must accompany minor child
  • There may be a nominal fee for some services
  • A $3.00 donations is requested for each vaccine given, but no one will be refused vaccine due to inability to pay.
  • Tetanus/Diphtheria boosters for adults
  • TB skin testing (Mantoux) is available by appointment
  • Provides all recommended childhood immunizations
  • MA/BadgerCare Accepted

  Vaccines for Children
Children through 18 years of age who are: 
  • Medicaid eligible
  • Uninsured
  • American Indian or Alaska Native
  • Underinsured (has health insurance, but the coverage does not include all vaccines or only covers selected vaccines.)
 
Fluoride Supplements 
Fluoride supplements are available for children ages 6 months to 16 years old who have well water at their primary residence. Supplements are also available for those children living in the communities where the water supply is not fluoridated. Supplements are recommended to decrease the incidence of tooth decay in both the baby and permanent teeth. Fluoride supplements are provided in liquid or tablet form and are taken daily. The Adams County Public Health s asks for a $1 donation.
